We are excited to have your child registering for Blizzard. Please complete all the information requested on this form. We will still need you to sign and date the medical release form. Also, we do not accept electronic payments at this time. You may use automatic bill pay, mail a check, or turn in your payment. Please send your child’s $25 deposit or camp payment made payable to Everett Family Church.

Mail payment to 2413 98th ST SE, Everett, WA 98208 or turn into your church’s Blizzard contact person.

If you find it easier you may simply print out the Blizzard Registration form and mail or turn it in with your deposit.
